The Shared Med Journal is a free app which allows you to create as many medical journals as you need to manage the healthcare of yourself and your dependents. Each journal can be shared with others, including Healthcare Providers, to form a Care Team for that particular Journal.
You can think of the journals you create as highly secured, encrypted, private groups with a timeline visible only to the Care Team members invited to it.
Each journal contains a place for storing medical history and posting updates and photos. All Care Team members invited to a particular journal are able to monitor, post and comment on entries in that specific journal.

Local data is password and passcode protected
If the Shared Med Journal app has been prescribed by your doctor, it will include details of the doctor and their practice for easy reference.
